Temporary Worker Starter Declaration

Tick one of the following three statements

Statement A

This is my first job since last 6^th April and I have not been receiving taxable Jobseeker's allowance, Employment and Support Allowance, Taxable Incapacity Benefit or an Occupational Pension.

Statement B

This is now my only job but since last 6^th April I have had another job, or received Jobseeker's allowance, Employment and Support Allowance, Taxable Incapacity Benefit. I do not receive a state or Occupational Pension.

Statement C

As well as my new job, I have another job or receive a State or Occupational Pension.

Loans

I have a student loan which is not fully repaid and I left a course of UK higher education before 6^th April and I receieved my first student loan installment on or after 1^st September 1998.
